Draw this shape with pen tool :-

Adjust the nodes as shown :-

Apply these layer styles in it: -

Draw this shape with rounded rectangle tool having radius of 5 pixels :-

Apply similar gradient overlay in it. In addition, slightly change the inner shadow settings as shown :

Draw this #91C360 color as shown :-

Create a new layer. Draw this shape :-

Finally, reduce the opacity of this layer to 33%. Our glossy flask icon is ready :)
